Creating Exam and Survey Questionnaire (including Feedback activity)

Step‑by‑step Instructions
Sign in to your Moodle course
- Sign in to your Moodle account using the provided link. Enter the username and password that were provided to you.
- Click your name in the top‑right corner.
- Click the course you want to use.
- Click “return to course.”
- Enable course editing mode near the top‑right. Editing may already be enabled; in that case, it will display “turn editing off.”
Create a question bank for the exam
- Click Question bank in the left tab of the course.
- Select the question category and click Create a question.
You can also create question categories to organize your questions. For example, if a course has several exams, it is useful to have a question bank for each exam. - Choose the desired question type and click Next.
- Enter the question name and the question text.
- Set how many points the question is worth. Enter the number of points for the question (normally 1). For multi‑answer questions, you may assign 2 points or more if appropriate.
Note: The remaining steps vary depending on the question type.
Create an exam
- Click Add an activity or resource at the bottom of the chosen section.
- Select Quiz/Test from the options and click Add.
Create a survey
- Click Add an activity or resource at the bottom of the chosen section.
- Select Survey from the options and click Add.
There is no question bank for a survey; it is a general feedback form with multiple‑choice responses students provide about the course, e.g., “How did you find the course?” choices: very interesting – interesting – not very interesting – etc. Results are tallied.
Create a Feedback activity
- Click Add an activity or resource at the bottom of the chosen section.
- Select Survey from the options and click Add.
- Add the activity name and a description of your choice. Afterwards, configure the necessary settings as needed.
- Open the activity on the course page by clicking it.
- Click the Edit questions tab.
- Select the question type.
Tips / FAQs
- Create separate question categories per exam to keep the bank organized.
- For multi‑answer questions, adjust the point value to match your grading scheme.
- Surveys are for general feedback; they do not use the question bank.
- After creating the Feedback activity, review its settings before publishing it to students.
